Something about human state machines within organizational ones:

Compounds exist in the air of your home - some are identifiable pollutants.  Some are a mystery.  The mystery compounds in the air are often the compounds that result when pollutant A reacts with pollutant B to give you something that was not on the list of things you expected.  Some of those can be quite reactive with your body, and that is generally not a healthy thing.

People have communications issues between one another.  Some communications problems are due to identifiable patterns in individuals.  Some are a mystery.  The mystery problems are often compound problems that result when the communications problems of person A react with those of person B to give you something that was not on the list of things expected.  Some of those can be quite reactive with the rest of an organization, and that is generally not a healthy thing.

For instance, if person A is incapable of admitting they are ever wrong, and can never apologize, and person B incites people to anger, then the organization will develop a long-standing issue if B incites A to violate rules of common courtesy, and A cannot apologize.

Person A might only exhibit their problem in certain mental states.  The rest of the time they might be capable of apologizing.  Person B might also only exhibit their problem in certain states.

A and B, therefore, will manifest their long-standing issue only if they interact in the states that catalyze their deadlock.

Of course people are not simple state machines, they are ridiculously complex ones.  I'm not sure that psychologists analyze people as linked state machines, but there are certainly great tools available to them should they choose to do so.  k-maps could be applied to find race conditions and boy, would you have a lot of tools for finding and solving deadlocks.
